| 1. Make sure you have your 5-digit pin number. |

If you did not get a pin number when you signed up, you can get one if you:

a. either print, fill out, and drop off the PPV signup sheet
|
|

b. or stop into the Conneaut Telephone Company to fill out the PPV signup sheet |

| 2. Build a list of channels and select the ones to restrict. |

|
|
| 3 program the restrictions list by calling the parental control system at 599-6900. |
|
a. Enter your 10 digit home phone number, including the area code
(for example: 4405930000).
b. Enter your 5 digit pin.
c. Choose option 3 to activate begin building your restrictions list.

|
| 4. Now that you have the list built, you can call 599-6900, and choose option 1 to restrict the channels, and option 2 to enable the channels. |
